Abscesses are generally due to some sort of local infection .Diabetics get it more as a result of decreased immune status. HEART BURN/WALKING/YOGA

Now-a-days  focus is back on the age old and proven Yoga remedies for some of ailments.Yoga to me is a form of physical activity closely related to exercise.And as we understand walking is best exercise.I personally have experienced the effect in two of my discomforts. I had severe heartburn (occurs due to acid eructation) and irritable bowel problem.Heartburn  sometimes occurs due to lax muscular guard called lower esophageal sphincture. This sphincture prevents the reflux of stomach contents obviously mixed with acid present in the stomach. I propose walking causes rapid excursion of the muscular divide situated in between chest and abdomen called diaphragm. This diaphragm forms major part of the lower esophageal sphincture. So exercise of this muscular structure like any other muscle tones  it up  and in turn the sphincture to prevent  the reflux. Rapid inward and outward movement of abdomen also massages the gut and helps in normalizing it's movement.Perhaps that is why I got relief from these two discomforts .

FIBRE/VEGETABLE IN DIET/COLON CANCER.

Recently many things are told and advices are given on diet. The most important,cheap and readily available is vegetables. The most important content those help are fibre ,Vitamins and water content in it. Fibre is the building block of the vegetables and are non digestible in human beings. It is also present in whole cereals.These fibres retain water and form bulk of stool, so that constipation is avoided and emptying time of  stool in gut  is   increased/optimized.This in tern helps in early elimination of harmful chemicals from gut and results in less contact period with gut interior.It is known that excreted chemicals from metabolism and those formed in the process of digestion if remain in contact for longer period induces changes in the inner part of gut called mucosa.

Early elimination helps in preventing colon cancer,irritable bowel syndrome.On the other hand red meat causes constipation resulting in  longer period of contact of chemicals with gut mucosa and initiating  cancer process.

DIABETIC FOOT CARE

Diabetes mellitus is systemic disease with far reaching adverse effect on all most all the systems of body. More often  diabetics get foot ulcers and if neglected ,end up in loosing the foot.Diabetic foot care is a vast subject for discussion.Here are only the gists. Diabetes especially  if un-controlled/under controlled, invariably affects the nervous system and vascular system.When the nerves carrying sensation are affected the patient does not get pain from a trivial/un-noticed injury and remains un-noticed.If the blood supply system is also affected less/no blood reaches near the wound affecting the healing. It may grow  out of proportion and culminate in loss of the part.

So it is advised  to prevent the process first by (1) adequate control of blood sugar.(2) Use of fitting and soft foot wear(ill- fitting shoes cause friction and may lead to ulceration) even in the residence.(3) Careful enough to see, if there is any loss of sention in foot preferably each night while going to bed.(4) Also inspecting for un-noticed trivial injuries if any.(5) Washing  the foot before going to bed with luke worm water removes dirt.

WINDOWS INTERNET FXPLORER/DELETION/RE-INSTALLATION/HEADCCHE

I most of the time set Firefox as my default browser.At some point of time while unsupervised  updating of internet explorer it has been set as default browser. This option of setting default option comes during installation.When I noticed it I wanted to change it.Being a surgeon and novice in computer use I went to the menu from start and could find the option of setting the default /access program .After opening it there was option of default/custom for microsoft and non microsoft program .I instead of going to custom checked default/non microsoft  and selected firefox as default and allowed to configure. To my utter surprise till I understand what is going on my IE8 was deleted.I felt bad and wanted to configure it again from the set up already downloaded and  preserved in my system.  Then the horrible happened in form of non configuration /no icon etc.of IE8. I thought it might be due to no previous version in the system. So I un-installed IE8 whatever I call half installation , downloaded IE7 and tried to configure . My system was Windows XP Service pack3  and again it did not configure IE7. So as a desperate measure I restored the system back and found my OLD IE8 and relieved. CULTURE AND LIFE

Recently I had an opportunity to interact with a group from Mangalore.Out of curiosity I asked them about corruption in public life and about the recent Pub attack there. The reply to corruption was generally yes and more so in the office goers of PSUs. They disapproved and are against the vulgarity in pubs and hegitantly disapproved the assult.Again they expressed their anger for in-equality in treatment of crimes on the basis of religion.Also they emphasized that the incident was blown out of proportion for vested interest of some groups. I concluded that the locals are against vulgarity on the name of recreation.But whatever happened affected health of persons insome way or other. Do you agree?

BLOOD SUGAR ESTIMATION dos and donots

BLOOD SUGAR RATHER TO BE CALLED PLASMA GLUCOSE.
A few things are to be kept in mind to go for next day assessment of plasma glucose.One has to remain fasting for 8 to 10 hours overnight after taking usual diet in the night for this,may take one glass of water in the morning. In no case less/more food should be taken which may give a wrong reading.If taking insulin or drugs for established DM should also take those as usual at night.Then go for giving bloodin the monnig . After that for postprandial plasma glucose estimation normal/usual diet for lunch to be taken along with insulin/oral drugs if any and blood should be given 2hours after .Always venous blood, preferably from the vein in elbow should be given.If all these aspects are not looked into one may get a fictitious result.

ABC AND D OF FIRST-AID/PUBLIC/POLICE/HOSPITALS

First-Aid to the victims of accident of different types viz. road traffic accident,fire and drowning etc. is a life-saving measure and a fortunate few only get it.
So we call it A,B.C and if not given then D.
A---Airway cleaning. And it is the first in the hierarchy.Air way i.e. throat of the accident victim if clogged with blood ,saliva,mud or shoot from burn or any other substances should be cleaned thoroughly so that the victim can take respiration and choking of airway is prevented. This is because a person may not survive more than 3 minutes without oxygen.
B---Next thing is breathing .If not taking respiration artificial respiration in form of mouth to mouth should be started till respiration is established.
C---Next comes circulation.That means external bleeding should be secured as far as practicable in form of applying pressure bandages.And in suitable situations placing an intravenous line.
D--- If these aspects are not looked into the patient may land in DEATH making US responsible for it.
To-day especially in urban area many do not do any thing seeing an accident victim for many reasons and one being police hassle. I read somewhere that there is a law in China that the commuters must act in such situation or else liable for prosecution.But in Our country India much is done on humanitarian ground.I think police has a role in educating the people about flexible legal provisions in case of accidents.
As regards treatment according to The Apex court ruling any hospital including private ones has to manage these patients even though the patient has no money at that time to pay.

GOUT

Gout is a form of joint disease characterized by excruciating pain especially in the small joint of foot.Typically it affects the joint of great toe called the metatarsophalangeal joint which is swollen,glossy,reddish and painful to move. The pain typically occurs at night is excruciating in nature keeping the patient awake and seek help. Commonly it affects persons with high animal protein intake apart from various other causes.A sort of metabolic product uric acid gets deposited in the joints.The disease can be confirmed by testing the uric acid level in blood which should be above 8mg.% in male and in female above 6mg.%. Medication shold be continued basing on advice of a physician and it remains under control.

SPRAIN AND MASSAGE

It is often noticed that people specially in urban areas embark upon massaging pain killer creams/oils etc on the swellings caused due to sprain or trauma.And that is wrong.The swelling occurs on the site as a result of tear of minute/small blood vessels followed by body response called inflammation.If it is massaged there is every possibility that more blood vessels will be affected leading to more swelling and pain.The ideal is to apply ice over it for sometime and repeating 3/4 times a day for 4/5 days ,along with rest to the part.Some times a broken bone may be found on X-Rays examination of the part.If so expert advice should be sought.
In my childhood I could member household remedies like putting a wet cloth over it , application of some short of small leaves and of course ,rest.I think that is something to-day we advocate.Is to-day's massage is due to advertisements and overlooking ancient practices?

Thread worm infestation

Thread worm infestation is a menace in a few countries like ours India .It mainly infests the intestine of children .The root of infestation is through oral in take of food and drink contaminated with it's egg.The worm develops and remains in the intestine only to come out to anal region to lay eggs at night. That in is why it gives an itching sensation at night and contaminates inner wear. The special thing about it's treatment is to wash the garments in hot water or hot press next day of taking drugs for it. I think a very few doctors/pharmacists give this follow up advice and it is essential.

BROADBAND INTERNET CONNECTION IN PCLINUXOS

Since being a surgeon and novice in use of PCLinuxOS, I struggled hard for days to configure and manage Broadband(Dataone) internet connection (India) and I finally succeeded this evening.I was configuring it opening control center~network and internet~removing connection~set up new ADSL/LAN~DSL~India~user name and password~Start from boot and manage ~finish.But it never connected from boot and I had to setup connection again from the beginning.I was bored and could not find any appropriate clue from the Google search.This evening I marked one icon in the lower right corner of the desktop on task bar .And Eureka ,there is the menu to connect after right clicking. I loved so much the graphics, speed and less virus attack features of it ,but was frustrated being unable to use.I hope my experience will help some people like me.

INGROWINH TOE NAIL

I think a few know how to cut the nails. It is often seen that patient come to surgeon with infection in the corner of the nail bed and the edge of nail deep inside the nail bed. The nails in these cases are usually cut in a semicircular fashion and that helps the nail to get buried inside and acquire infection.The correct way to cut the nail is to cut it straight a little away from the edge of nail bed so that the corner of nail does not go inside. The treatment for this is either partial or full removal of nail under local anaesthesia. So why not to prevent it ?

SURGEON AND SOCIETY HEALTH

A patient is sometimes surprised when a surgeon instantly agrees to take up his case .Generally speaking, it is believed that surgery is never done free of cost even in Govt/PSU run hospitals without some sort of illegal gratification. And this surprises the patient.But this belief is not always correct and is due to general run down in the moral health of the society. There are odd ones also in this age of majority .

I got it downloaded ,installed in a partition.But one thing I face a problem is in configuring the Broadband ,of course in India. May be there is some easy way out not known to me. But ISP configuration is definitely not easier than Ubuntu.The graphic is good as well as the speed.

EXERCISE/SEDENTARY LIFE STYLE

Exercise for those pursuing sedentary life style especially after 35yrs of age is a must to keep them in good shape and prevent lots of diseases.The diseases progress as a chain reaction i.e. obesity~diabetes~hypertension~kidney diseases~eye diseases and so on and so forth. Amongst exercises of various types brisk walk is very much accepted and recommended. A brisk walk means speed of at least 7-8 kms/hr and for at least 45 minutes for 5days in a week.Researchers say that this exercise tunes up all most all the muscles of body ,cardiovascular system .If a little diet modification is added ,it works wonder by making one very much trimmed and fit.Researchers say it reduces/prevents weight gain ,diabetes,hypertension and boosts body defense system.So why not to spare some time in keeping oneself healthy and away from doctors.

ANXIETY

The anxiety of undergoing next day surgery has to be alleviated at present by the surgeon only .Anxiety causes loss of sleep and apprehension. Good pre-operative counselling of the patient and adding some anti-anxiety drugs will do the work.In this regard the work of medical social worker and para-medical staffs is essential. And till now this aspect of appointment of counseller in many hospitals has not been looked into.

SURGERY/SURGEON/PARAMEDICS

Preparation of a patient for surgery is generally done by paramedical staffs and they are well versed in it in a hospital.Surgeon thinks surgery is a holy work and comes prepared for surgery .If he sees the patient is either not prepared or under prepared it irritates. This occurs mainly in Govt./PSU run hospitals and is largely due to lack of attitude , assured job security and lack of supervision/facilitation by the competent authority.This need to change.

URINARY BLADDER DRAINAGE

Sometimes it is required to drain the urinery bladder through a created openingin the lower abdomen called siprapubic cystostomy by a tube called catheter .That has to be changed at least in every 21days if continuous drainage is required otherwise concretions get deposited on the portion of catheter that is inside the bladder rendering it blocked and difficult to remove.

HERNIA

Hernia is protrusion of contents inside a cavity such as abdomen through an weakness or natural opening in it's wall.Commonly it occurs in the inguinal region so the name inguinal hernia .Before embarking upon surgery it is to be ascertained whether some other disease compelling patient to strain viz. disease of prostate exists. Hernia is to be treated early to avoid any emergency situation that may arise.

Hydrocele though thought to be a simle to diagnose and treat disease should be examined carefuly to exclude though the less frequent but more serious testicular cancer.

Piles

There might be something more serious inside showing symptoms of piles i.e. painless bleeding per anum.Doctor should perform a per rectal examination.If not patient should insist for the same.
