Young adolescent boys and girls sometimes get pain in the right lower abdomen that may confuse with an acute appendicitis.Though there are several causes mesenteric adenitis is of particular interest.There is pain,vomiting  and sometimes fever.On examination of abdomen the site of pain can not be accurately located.Patient sometimes indicate a particular point of pain only to show a fresh point at some other time.The location of pain changes with the change of position of patient.That is the catch point.
  Mesentery is the attachment to the gut that holds it suspended inside the abdomen.There  are lymph nodes within it which collect nutrients from gut after digestion of food.Sometimes they get infected and become a source of pain.As mesentery is a long structure and hangs with certain degree of mobility the location of pain keeps on changing with the position the patient adopts.
  This can be differentiated from appendicitis with it's characteristic pain pattern and an ultrasound may demonstrate a healthy appendix.Sometimes it is hard to differentiate it from appendicitis in spite of all available investigations. In case of doubt a laparoscopy will be able to differentiate it from appendicitis.If still in doubt an appendicectomy is done which many times cures it.It just like two in one treatment.If correctly diagnosed a simple conservative approach by the way of parenteral feeding ,antibiotics  and analgesics will cure the condition .

The right lower abdominal pain that very closely mimic appendicitis is a torsion of right ovarian cyst.Ovarian cyst may occur in either side but of particular interest is the right side one.
  These cysts may be of benign or malignant in nature .They vary greatly in size.The interesting fact is that as there is no time for this torsion to take place it becomes difficult to differentiate it from an acute appendicitis.But a pelvic examination by an experienced gynaecologist will reveal the actual nature.An ultrasonogram will clinch the diagnosis.
    Once diagnosed, an expectant treatment usually followed in the hope of un-twisting of the the ovary with the cyst.If malignancy is suspected more tests in form of carcinoembryonic antigen are to be done  along with a CT scanning.If it seems to be benign but not responding to conservative treatment a surgical option is explored and best so far is a laporoscopic approach.

                       Right lower abdominal pain in ladies may come from an infection in the fallopian tubes .Fallopian tubes are simply called tubes which are connected to uterus in the middle  and freely open in the outer sides to receive an ovum.These are also called salpinx.These many a times get infected from outside through vagina and uterus .Some times gets infected from blood borne infection from some other infection.This infection is usually bacterial or bacteria like organisms and called Salpingitis.If it is in the right side may confuse with an appendicitis.It may also occur both the sides at the same time.The patient complains of pain in the middle to lower abdomen ,fever vomiting etc..

  In that case though there is some confusion still clinical differentiation is possible.This pain and elicited pain on pressure is more towards the center and in appendicitis it is more towards out side.The sequence of a classic appendicitis e.g. pain, vomiting and fever pattern is not present.A history of  vaginal discharge,  usually foul smelling may be present.A  pelvic examination will differentiate it.A swab collected from there will prove it pathologically.An ultrasonography will aid the diagnosis.
   So here a non-surgical treatment is followed and   a gynecologist takes over the case.

     Young ladies some times complain of pain in the lower abdomen during menstruation.If it is in the right lower abdomen confuses with the common and dreaded ailment in that site,appendicitis.It is again difficult and at the same time easy to distinguish.It is a condition called A Ruptured Chocolate Cyst of Right Ovary.This cyst may occur in the either ovary and right side is important for us.Other innocent cause is dysmenorrhoea which is almost a self limiting condition .There are some other causes of academic interest.We will discuss chocolate cyst of ovary here.

A chocolate cyst,the name is due to collection of of chocolate colored fluid in one or both ovaries.This occurs due to a condition called endometriosis where an inner lining of uterus called endometrium is found embeded in the ovary which is an ectopic site for that.There are so many ectopic sites for endometriosis.And is pathological.So,when menstrual cycle begins with menstrual bleeding due to shredding of endometrium in uterus also the same thing occurs in the other ectopic sites.And if an ectopic site is ovary there occurs bleeding.Which creates a space for itself and so become a cyst containing blood and some endometrium.This goes on in a cyclical manner with ever increasing size.Ultimately it can not accommodate more and bursts.
  It causes pain in that side of lower abdomen ,vomiting and some times fever.And if it is in the right side teaches the surgeon a lesson in excluding it from appendicitis..But most commonly this ruptures around menstrual flow time though can occur at any time.An ultrasonogram is helpful besides the clinical examination.

  If conformed conservative management i.e. without surgery will resolve the condition.But some times surgical intervention is required. Laporoscopy is better idea over open surgery as at the first place it will confirm the actual condition basing on which further action can be taken.

Abdominal pain that to right lower abdomenal and again in young ladies poses much difficulty as well as ease.It commonly confuses with appendicitis.Last time we had a post on ruptured ectopic pregnancy a differential diagnosis of appendicitis and how it could be distinguished .Another common cause of such type of pain is pain in the mid-menstrual cycle i.e. pain around 14 days before beginning of menstruation.It is called Mittelschmerz.
 This occurs due to rupture of follicular cyst during ovulation   associated with some amount of bleeding to the pelvic cavity.So, if it occurs in the right side confuses with appendicitis.Main distinguishing point is it's typical mid-cycle timing ,a negative pregnancy test and absence of typical rigidity in the right lower abdomen.
  Again it can be confirmed from a sonogram .It does not require much treatment except some analgesics and reassurance.Some times it repeats in some ladies.

The cause of abdominal  pain is many a times very difficult and some times very easy to diagnose.If a lady of reproductive age comes for right lower abdominal pain it makes more difficult  to arrive at a definitive diagnosis.There are number of diseases which present as right lower abdominal pain and in ladies more conditions add up.But a married lady of reproductive age presenting with missed period ,right  lower abdominal pain,severe pallor and shock the most probable diagnosis will come to mind is ruptured ectopic pregnancy in right side.
Ectopic pregnancy is implantation of product of conception in a place other than the normal i.e. uterus.There are several ectopic sites .The most important and common site is tube.Even ectopic pregnancy is seen in women on temporary contraceptive measures like intrauterine devices.This happens because after the fertilization in a part of tube the product does not descend to the normal site i.e. uterus and gets implanted in a part of tube.As the product grows in size, the narrow tube can not accommodate it and bursts towards the end of 8th week to the   beginning  of 12th week.When this happens it is a medical emergency.There is profound blood loss in abdomen and all other symptoms as described earlier appears.
It confuses among other with most common appendicitis and distinguished by profound pallor.An ultrasonogram clinches the diagnosis.Intervention at a correct time saves the life of patient.

My friend in PeerPower.com has sent this message to me requesting for wide circulation.I would like to add a few things a few here.As regards the mask and precautions he is right.But using a mask is better to no use.What I called something is better than nothing.Moreover it will obstruct other secondary bacterial invasion to cause a pneumonia in the already immune compromised patient.It may be helpful to the patient if not to the contacts.Tamiflu is a  virostatic drug which will prevent rapid proliferation.I can conclude that confining the less sever cases to home with usual precautions is a good strategy.And sever cases be treated at hospital.

Dear All
A very logical and constructive solution to prevent you from getting infected with H1N1..Plz share it to all friends and relatives.
The hype in media about the utility of face masks and N95 respirators as a tool for general protection against H1N1 can't be deplored enough. Please realize that this is not an official advice, especially the one about face masks or N95.
·Most N95 respirators are designed to filter 95% particulates of 0.3µ, while the size of H1N1 virus is about 0.1µ. Hence, dependence on N95 to protect against H1N1 is like protecting against rain with an umbrella made of mosquito net.

·Tamiflu does not kill but prevents H1N1 from further proliferation till the virus limits itself in about 1-2 weeks (its natural cycle).
H1N1, like other Influenza A viruses, only infects the upper respiratory tract and proliferates (only) there. The only portals of entry are the nostrils and mouth/ throat. In a global epidemic of this nature, it's almost impossible not coming into contact with H1N1 in spite of all precautions. Contact with H1N1 is not so much of a problem as proliferation is.
While you are still healthy and not showing any symptoms of H1N1 infection, in order to prevent proliferation, aggravation of symptoms and development of secondary infections, some very simple steps - can be practiced:
1. Frequent hand-washing (well highlighted in all official communications)..
2. "Hands-off-the-face" approach. Resist all temptations to touch any part of face (unless you want to eat, bathe or slap).
3. Gargle twice a day with warm salt water (use Listerine if you don't trust salt). H1N1 takes 2-3 days after initial infection in the throat/ nasal cavity to proliferate and show characteristic symptoms.
Simple gargling prevents proliferation. In a way, gargling with salt water has the same effect on a healthy individual that Tamiflu has on an infected one. Don't underestimate this simple, inexpensive and powerful preventative method.
4. Similar to 3 above, clean your nostrils at least once every day with warm salt water. Not everybody may be good at Jala Neti or Sutra Neti (very good Yoga asanas to clean nasal cavities), but blowing the nose hard once a day and swabbing both nostrils with cotton buds dipped in warm salt water is very effective in bringing down viral population.
5. Boost your natural immunity with foods that are rich in Vitamin C (Amla and other citrus fruits). If you have to supplement with Vitamin C tablets, make sure that it also has Zinc to boost absorption.
6. Drink as much of warm liquids as you can. Drinking warm liquids has the same effect as gargling, but in the reverse direction. They wash off proliferating viruses from the throat into the stomach where they cannot survive, proliferate or do any harm.

    Some times some people do not avail a chance and let that go. A folk tale in Orissa,India describes how people ignore the opportunities.Once "Lord Shiva and His wife Goddess Parbati"were in the sky to observe the people of earth.They saw a poor man in skeletons begging for food but was not getting anything.Parbati became felt pity for him and asked Shiva to see this and also asked why people created by them should suffer like this.God replied to it with a smile that some do not like a grab an opportunity and are lazy ,consequently suffer.And He wanted to show that to Goddess Parbati then and there.The god put a pot full of gold coins on the road of the beggar .But the beggar at that moment wanted to mimic a blind man and walked closing his eyes.He gave a kick to the pot and walked  past  to open his eyes later .Consequently he missed the gold filled pot.Then the god asked his wife to interpret it and they went on the voyage.

    Something of the sort happened to-day in our hospital.A person injuring his Achilles tendon came to me.Since ours is a private hospital only for port trust employees ,other people have to pay for the service with  a token price only.I inspected the injury and asked him to deposit the required fees to get the tendon repaired .But the person expressed his inability to pay.I suggested him if he does not have the money he can pay it later also.He was not willing for that also.But on humanitarian ground I offered him to repaired it free of cost using material we get as physician's sample but he has to get admitted to the hospital for some time.And to my utter surprise he declined for that and went away. Instantly my staffs reminded me of the story I just narrated.
   The thick tendon through which the calf muscles get anchored to the heel bone is called Achilles tendon named after the Greek mythological hero Achilles.This is a tough tendon in the body.This tendon lifts the body weight  off  the ground while walking.
   This tendon gets severed mainly by accidents,assault or in collagen diseases.When partially torn the person can walk limping and disability is more severe if completely torn.The person can not stand on toes of the affected foot.And if completely severed a distinct gap can be felt over an intact skin or the end can be seen if skin is  cut.The upper end of severed tendon usually is dragged by the calf muscles to an upper level and may not be seen.Tendon repair is a special procedure and done under aseptic conditions .It can be performed under local.regional or general anaesthesia.The repair is done using a non-absorbable suture material.After repair a plaster of paris cast is given to immobilise the foot for 6 weeks.After that period  a gentle physiotherapy makes the patient fit for duty.The results are good.

Pain by the outer side of wrist commonly seen in middle aged women more often than their male counterparts.Medically this is a sort of disease to spot the diagnosis.The affected person is unable to hold something tightly.If tries to do that gets pain along the muscle of that side of forearm.It is painful  if pressure applied on that point.
  This is called De Quervain's disease .This is a sort of entrapment of the pulley called tendon to the thumb responsible for outward movement of it.It is diagnosed clinically basing mainly on two tests.First there is pain on pressure over the outer side of wrist where the bone called radius ends.In the second test patient gets pain when asked to clinch a fist with thumb pressing  all other fingers and patient asked to deviate the wrist outwards  away from the side of pain.That puts unusual stress on the outer muscle and patients gets pain.
   The treatment comprises rest to the wrist,moist heat application and prescription of simple analgesics.If no response is seen local infiltration of steroid is done.Usually that cures  many but some times a second or third injection may be needed in a few patients.The side effect being discolorization of skin in some, otherwise it is effective.
  The last option is surgery intended to release the entrapped tendon.Here care is taken not to damage the superficial small nerves to skin and minute blood vessels to the finger.The limb is immobilized for sometime, antibiotic and  analgesics  are prescribed for a few days.Hand is kept elevated to reduce pain.Early physiotherapy is instituted after removal of sutures.And the De Quervain's disease is gone.

Finger Clicks on Clinching Fist

There is a clicking sound while clinching fist and pain along a finger of hand. This is a sort of spot the diagnosis for medical persons. This is a condition involving flexor tendons of fingers. There are tunnels and tendon pulleys in the fingers, made up of collagen/fibrous tissue .The tendon pulleys pass through the tunnels and work to flex the fingers while forming a fist. If the tunnels constrict or the pulleys thicken they can not pass smoothly and while passing along the deformed part it passes with a clicking sound .And hence earns the name Trigger Finger. If it progresses at one stage the pulley does not pass, so the finger can not be bent.

It may occur as a part of a collagen disease or without a specific cause. At the preliminary stage I advice the patient for physiotherapy under warm saline water. It relieves of the symptoms many a times. If not I infiltrate local steroid and lastly go for surgery to release the tendon pulley .The surgery gives immediate success. Under a local anaesthesia and skin crease incision the constricting portion is divided to release the tendon .Antibiotics and rest prescribed for some time.After removal of sutures a physiotherapy is advised.

Food grains such as groundnuts,maizes and parboiled rice may get infested with fungus.This, if occurs because of Aspergillus Flavus poses a health risk.One toxin called Aflatoxin is produced by it.Aflatoxin B1 is linked to serious diseases and deaths in India .Ingesting these contaminated grains may cause serious liver disease i.e. hepatoma, a cancer of liver or cirrhosis of liver.
Increased moisture and humidity facilitates fungus growth.The food grains must be stored dried and moisture content be below 10%.If contamination is noticed the food should be discarded.

Urinary Tract Infection

    The burning sensation and severe urge for urination  is very much distressing  for of  a patients of urinary tract infection.Urinary tract stretches right from the kidneys to the urethra.Any part of it may get infected.It is mostly bacterial in origin.But may be due to TB,Sexually transmitted agents and other systemic diseases.Infection may come from local surroundings or from systemic infection.
  The proximity of urethra to the anus makes it vulnerable to contact infection from the bacterias present in faeces.It affects females more as  the urethra is short i.e. only four centimeters in comparison  to males where the length of urethra is nearly twenty centimeters.
   Once infection sets in there is fever,burning urination ,scanty frequent urination and painful urge for urination.Infection may complicate an already existing  urinary stone disease.Or may set in an anomalous urinary passage.It may also set in a disease condition causing stagnation or obstruction to urine flow i.e. enlarged prostate or stricture of urethra.

  Diagnosis is not that difficult if proper history is taken.Mainstay of investigations rest on a routine and microscopic urine  examination,a culture and sensitivity test to know the type  of infection and appropriate antibacterial agents required .Investigation like ultrasonography is done to rule out any other associated or facilitating diseases.Other blood tests to know diabetic and kidney status are also performed.
   In ladies a little prolonged antibacterial treatment is needed as compared to the males where eradication of microbe is a little bit difficult. Diseases   complicating urinary infection or as a result of it need to be treated in it own merit.

       Brave & mighty rules earth.And that is also true in case of fertilizing an ovum ,of course in human.Millions of sperms rush to fertilize one ovum and one succeeds.Obviously  this is an arrangement by nature to have the fittest sperm take part in reproduction.Many a times male fails to fulfill the obligation resulting in male infertility.
   There are a number of causes for it.There may be abnormality in sperm production,transport,fertilization and above all timing to be able to meet the ovum in female partner.The quality of sperm has certain parameters such as 1)Quantity-2-5 ml/ejaculate 2)Number-60-150 millions/ml 3)Morphology-No structural defect at least in 80-90% 4)Motility->95% motile & 5)Fructose content-150mg% 6)Liquification Time 5 minutes. .These are to be met.
  Deficient production may be due to testicular defects such as 1) abnormally placed testis 2) defective or

atrophied testis etc.The testis develops inside the abdomen but come down to house in the scrotum before birth.Because it requires some sort of air-conditioned atmosphere to be able to produce sperm.And the arrangement by nature is in such a way that scrotum provides that .The scrotal temperature is less than abdominal  temperature and that is favourable for working of testis.And if for some fault testis can not descend down as in undescended or impefectly descended testis the functionality decreased or abolished.This air conditioning mechanism may be defective if there is a hydrocele where fluid surrounds the testis or a varicocele where dilated blood vessels surround it resulting in bad sperm formation. .

    Trauma to it,infection with mumps/syphilis or diabetes mellitus may affect sperm production.There may not be testis at all in some and may be rudimentary in some other.A careless radiation dose may damage the functionality of testis. An abnormality in the male organ must be looked into for possible cause.
    And last but not the least timing with ovulation in female must match .

   So in this background the person must be assessed to arrive at the cause of infertility.First of all is the semen analysis which should be performed  at least after 72 hours of abstinence.If it is not within the normal parameters or   no sperm in ejaculate a biopsy of testis should be performed to know the status of structural integrity of testis and vas is to be assessed for possible blockade.If  semen analysis is normal the timing factor is to be looked into.Ovulation usually occurs around day fourteen of a 28days menstrual cycle.And that time is to be targeted.
 If all of  the above found to be within standards then probably the female partner is to be assessed for..

          After creation of the universe , the God of Destruction "Lord Shiva" ordered His godly carrier a bull "Nandi" to go to Earth and convey the inhabitants that they will take food one time in seven days.Nandi went uttering and remembering the order loudly in the fear of forgetting it.But the worst happened , he reached the earth only to announce that Lord Shiva has ordered the inhabitants to take food seven times a day.And that was totally reverse.Then he returned to heaven to convey Lord that His orders has been communicated.Knowing the totally reverse communication God became angry and said that as the bull has done the mistake he will  do  the cultivation of land  for human to produce  the huge quantity of  food that will be required .And since then the bull is doing the hard work for mankind.
      Whatever may be the folk tale the fact is that human has got a good appetite for food.It may be lost in various occasions starting from a trivial fever to more dreaded  cancer.

          Loss of appetite is different from anorexia many times used as synonym.Loss of appetite means there is no feeling of hunger and anorexia means there is hunger but no desire to take food.One feels hungry by the site , smell or at the usual time of taking food.So it is essentially centrally controlled .There is a center for appetite and satiety in the brain which regulate intake of food.

       Apart from minor and temporary causes of loss of appetite ,there are more important causes for loss of appetite and anorexia.One condition called Anorexia Nervosa  seen in young and adolescent  ladies where there is anorexia probably due to hormonal imbalance.Here the patient does not take food and becomes bony thin.This gets cured by hormonal support and assurance .

     Next important cause is old age ,here the taste buds of tongue gets destroyed , there is no taste of food and consequently the person does not want to take food.Vitamin supplementation relieves it to some extent.
     Gall bladder disease,liver disease and disease of pancreas can cause loss of appetite.

   Loss of appetite with new found anaemia in old people points to cancer of stomach.Any cancer can be a cause  loss of  appetite.
  There area host of other causes.

                   Diagnosis and treatment depends on the cause.

Menopausal Woman & Osteopoosis

Breaking of the thigh bone & vertebrae especially in post-menopausal woman are seen more often in comparison to males of that age.Though rarification of bone called Osteoporosis plays a role it is not absolute cause in my opinion.Several panic buttons are triggered and volumes written about prevention of bone breakage in menopausal ladies.Those are worth reading .

   Osteoporosis occurs in those ladies who lead a sedentary life style.I have perhaps never seen a fracture of thigh bone in ladies who work in the field in my village. Some cases are seen in the urban set up.

   Osteoporosis occurs in the later life due to excessive activity of bone resorbing cells  called osteoclasts and decreased activity of bone forming osteoblasts probably due to decreased or weaning female hormone  activity.Add to it are Vit.D deficiency,lack of exercise and less dietary calcium intake.If some drugs like steroids are being taken for some disease that enhances osteoporosis  .Associated kidney diseases contributes to it.

   So different articles describe drugs those halt bone resorption and female hormone therapy for it.Female hormone supplementation has it's inherent problems of vaginal bleeding,facilitation of breast and liver benign tumor formation .And drugs targeting inhibition of osteoclasts are costly.

     In this setting a few tests described such as Bone Mineral Density (BMD) and Bone Mass Index(BMI) are worth to be taken up.But the abnormality if any should not panic one for searching of drug remedy.Because an osteoporotic bone may not necessarily fragile.
   Therefore it is necessary to take adequate weight bearing exercise,Vit.D,exposure to sunlight especially morning,nutritional supplementation for calcium and avoid unnecessary  use of drugs those contribute to osteoporosis.
   After trying all of the above one may opt for drugs those halt osteoporosis in consultation with their doctor.There are also some phyto-remedies available where it is claimed to contain phyto-estrogen without much side effects.Experienced people must be consulted to continue those.

Reeling Of Head/Cervical Spondylosis

Reeling of head is a distressing condition.In medical terminology it is called Vertigo.Here there is sensation of reeling of head or the surrounding.There are so many causes for it.It may be physiological or pathological. Mainly three systems are involved in it.The ear and related nervous system,the eyes and related nervous system or the peripheral sensory system.

Physiologically this is seen while suddenly getting up from a sitting or standing position called a positional vertigo.Amongst the pathological conditions atherosclerotic hypertension is most common .Other diseases of the three system described above can give rise to vertigo.Common causes are an infection or disease of the middle ear,visual disturbances like ill fitting glasses , sensory disturbance of lower limbs and more generalized diseases like diabetes are important.
Atherosclerosis causes impairment of blood flow to the brain and it's subsections causing vertigo.Any body presenting with vertigo should be examined for atherosclerosis first.Hypertension easy to diagnose should be treated.Investigations like blood lipid profile is contributory.
Ill fitting glasses easily be corrected and an ear disease is easy to diagnose and treat.
The next important disease to cause vertigo is Cervical Spondylosis.Here the seven neck vertebral bones get affected commonly due to aging process.Other diseases of neck bone like Ankylosing Spondylosis also

important.Here the vertebrae are deformed ,loose the flexibility and loose the natural C curve.The deformity may pose an obstruction to the blood flow in the vertebral arteries which travels through the bony processes of of neck bones.The vertebral arteries in tern could not deliver required amount of blood to the brain system and cause relative ischaemia and the consequent symptom of vertigo.This disease can also be diagnosed easily from a host of available investigations i.e. from a good quality digital X-Rays to MRI .



Adequately treated spondylosis can go a long way in relieving the symptoms.

But
before jumping to diagnosis of Cervical Spondylosis as cause of vertigo the more common and dangerous cause atherosclerotic hypertension should be excluded.
