MELANOMA/A SKIN CANCER

A somewhat aged lady, attending her doctor with a history of a long standing black mole in the exposed part of body which has grown in size in the recent past to bleed on touch accompantied by some pain and is angry looking ,perhaps is describing a malignat melanoma , a form of skin cancer.
This is the warning sign of transformation of an innocent looking black naevus to a malignat melanoma.This type of skin cancer usualy occurs in white population living near tropics having long standing exposure to the ultra violet rays. We have hard about ozone layer coverig the earth and protecting us from the harhful effects of ultra violet sun light.Also we come across the news of holes in the ozone layer.The people of areas under the ozone holes are most commonly affected though no area or race is immune to it.
There are five type of malignat melanoma some are more aggressive and some are least. If such thing is noticed first of all a tissue diagnosis is sought for in order to confirm the diagnosis from similar looking non malignat conditions.
Surgery primary mode of treatment. Chemotherapy and immune modulators are also helpful.The outcome is not that grim.

HUNGER

In Sanskrit it is said that " BUBHUKSHU KIM NA KAROTI PAPAM".This means hunger struck person does know what is wrong and what is right ,can go to any extent to do the sin to feed his family.
And that is what world witnesses in some parts of it,Somalia as for example.Most of hunger are due to laziness of the persons themselves. But some are beyond their control eg. war and natural calamities.In Oriya there is a proverb " PRAJARA DOSA RAJA MUNDARE" means the faults of people automatically goes to the king. That means the emperor is responsible for the plight of it's people. So in hungers which are beyond individual control are due to the king's fault.The citizen has to bear the brunt .
What needed is the futuristic and pro-active measures to tackle an anticipated situation.Famines can be controlled to some extent if not in totality.War can be avoided. But in case of internal violence in a disturbed state control of situation is very very difficult. I think that is the area where UN should come to the picture.But that may not be easily carried out due to the pressure of some selfish states.
I believe if the concept of one world one state is established which again I say is difficult but not impossible ,there will be no hunger.And that is to be made possible be The UN.

NEWBORN CARE AND UPDATING FIREFOX WEB BROWSER.

We could not made it possible to attend the welcome ceremony of the addition of a cute little to the family of my neice observed on 30th day of her birth.Thanks to the Tatamotors hospital who gave their final touch.The mega event had attracted a number of visitors.However due to some strange coincidence we attended her around 60 days of her birth.My surprise new no bounds to see that my neice is taking her care like a grandma.
Since the newborn human being is the most helpless creature of God needs utmost care starting from head to toe.i.e.care of hair,nose,mouth and tongue,ear,skin and especially the neck folds,nails,cleanliness and feeding etc.

Coming to feed there are so many talks,the best being breast feeding continued up to weaning.The first yellow milk colostrum, variously named in villages of India,, one being witch milk is usually discarded but this contains a host of immunity boosters to resolve a number security and defense related problems of the newborn. This is a must for any newborn.A scheduled feeding at an interval of about three to four hours till 10 PM can be intelligently converted to demand feeding.No milk secretion is some times a wonderful excuse for external feeding, should be discouraged.An additional benefit of breast feeding is that it prevents breast cancer in mother to a larger extent.If at all external feeding is sought the advice of your pediatrician is by far the best but as it is said in India, if you need mother can make cow your mother ( Cow=GOMATTA in Hindi.)which means cow's milk is the next alternative for baby.However it has to be diluted in different strengths according to age of infant.
After each feed the baby should be put on mother's shoulder and gently thumped to eliminate the gases inadvertently sucked in, so that distension of stomach does not occur.While feeding one should keep vigil so that no milk enters the ear to set a middle ear infection .If baby vomits the head should be turned to one side so that everything escapes out and not sucked into the respiratory system causing serious consequences.
Weaning is shifting to other foods from breast feeding is to be started as early as five to six month of age. Though important is not practiced correctly in India even in the educated ones due to various customs.The shifting to other food should be smooth starting from addition of cooked curry water and pulses, lightly cooked fruits such as apples,juices,boiled egg yolk to easily digestible rice and the baby should be sharing home cooked food by the age of 1 year.
Regarding food to be taken by mothers, in India even in these days so many restrictions are in vogue. Those are being discarded gradually in favor of an expert advice.
Applying certain things to the skin of newborns in India such as a black pigment called KAJAL (in Hindi) specially to the eye lids may prove deleterious.
Having said this I presumed that the delivery is a hospital one or supervised one.Because a lot of unhealthy practices are followed in rural India during delivery.
Apart from these immunization schedule is to be adhered to and the lapses due to various reasons should be brought to the notice of attending pediatrician for corrective measures.
I saw that my neice is much educated in baby care and may not need much advice. We enjoyed the stay very much in the place of my co-brother in Tatanagar. In this period I launched firefox web browser in their laptop.The version was 3.0.9 and to-day I find updates to 3.1.00.So it need to be updated.That is very easy .Just one has to look for help menu located right to tools menu above the space for url in the opened firefox web browser.On clicking it one will see updates and clicking that sets the process to motion.Following the instruction will complete the updates.

MILK EXPRESSION AND VAGINAL BLEEDING IN NEW BORN FEMALE .

Many times the mothers and their relatives notice expression of milk and vaginal bleeding in a new born female baby which make them rush to the doctor for clarification.These are natural phenomenon in a female baby and occurs due to withdrawal of circulating estrogen from mother shortly after birth.

Small reddish spots and some yellowish discolorisation of body are also noticed invariably in all the babies.These are natural,occurs due to break down of foetal haemoglobin in the circulation and subsides naturally.But if prolongs medical attention should be sort for.

VISITORS BEYOND VISITING HOURS TO HOSPITALISED PATIENT

It is from my experience , I can surely say that a few visitors are genuine in visiting a hospitalized patient. And they abide by the hospital visiting hours to a larger extent if not perfectly.In India specially in Govt./Public sector undertaking hospitals visitors usually crowd their patient as if there is no specified visiting hours and their absence will prove detrimental to the patient's treatment.I think they come to show their face to the patient that they have put their attendance.This intention has been proved in the sense that, if they are asked to help the patient, may be by donating blood,helping the patient in shifting to other departments for some purpose ,they do not respond and some will hide their face and fly away.Most of time they do a harm to the patient. Because the patient can not take rest if has been advised.Instead of remaining calm patient goes to tension is describing the same illness story to each new visitor. I think patient himself might not be in a mood to talk. but has to speak to keep the face of the visitor.In this part of our country this practice contributes to a lot of cross infection to the already immune/defense compromised patient .In my opinion this sort of practice should be stopped.Not that hospitals are there in India where this visiting hours is not strictly followed.
Now the latest mode of representation is in form of mobile calls because mobile phones are now rampant in use here.And this is more easier and cost effective mode of representation before the hospitalised patint.
CAN WE BE RESPONSIBLE TO SOME EXTENT IN THIS REGARD IN HELPING OUR NEAR AND DEARS TO EARLY RECOVERY?

WORNING SYMPTOMS OF HEART ATTACK

Generally speaking heart attack means assault on it's muscles.Heart can be thought as a muscular pump,maintaining blood circulation throughout the body providing nutrition and oxygen .The heart also get supply from it's own blood vessels to survive and function.Due to various reasons importantly blockade to it's blood vessels, may be partial or complete, deprives it of the oxygen and other things.This blocked most commonly occurs due to deposition of bad fat on it's wall called atherosclerosis . FRACTURE OF THIGH BONE(FEMUR) AND OTEOPOROSIS

The typical description an aged female patient and her relatives gives is that she fell down in the bathroom and unable to get up. The clinician finds a fracture of neck of femur, the upper part of thigh bone.And the book also describes in the similar manner.Actually the truth is that the bone breaks first and patient falls down.This is called a pathological fracture usually follows osteoporosis associated in the old age.There are other causes of a pathological fracture also.Estrogen a female hormone plays an important role in maintaining the bone density.After age of forty gradually the hormone secretion decreases resulting in more porous and weak bone, making it vulnerable to crack .If one follows weight bearing exercise, walking being one of them ,it helps to make the bone stronger by applying stress.Along with it she should take calcium and calcium containing food supplementation according to advice of doctor.It can be avoided to a larger extent if can not be stopped.

The new users of windows XP and PC seldom know that maintainence of operating system is required.When they start using are carried away by the advertisements for cleaning/defragmenting their operating system and run for downloading softwares from net like me. ANAEMIA

Anaemia is a manifestation of many diseases.In rural India most common causes are hook worm infestation and poor nutritional status. It is mostly seen in children and women.Hook worm resides in the upper part of intestine called duodenum entering body through foot(bare) following open air defecation. It take most nutrients which are supposed to be absorbed in duodenum. Among them iron being most important.It is an important element in formation of hemoglobin.It also feed on blood and injures intestine resulting in small bleeding inside.Now -a -days though the incidence is reducing after use of safety latrines but still registers its presence.Prevention is the choice.Early treatment saves five.

Uncontrolled and poorly controlled diabetics are prone for skin and soft tissue infection due to poor defense status and a host of other factors .In this part of our country diabetics are hardly conscious about it.Repeated advice in this regards at least those in public sectors does not get full result and what to speak of illiterate/poor mass.They do not go for blood glucose test in regular intervals only to come with carbuncles,furuncles and fungal infestations leading to resource crunch and loss of man days .

MYSTERIOUS LIFE/THE LIFE CENTER/LIFE AFTER DEATH/SOUL

As usual I started my morning walk at 4.45 AM setting my PC to download some recent updates for PcLinux 2009.1.I headed towards the seashore and the moon was above the horizon.I wondered about life and death.How can it definitely be defined ? Somebody ,of course human is said to be living when his heart continues to beat,able to take respiration,moves his limbs,talks and responds etc.But vital among them are activity of heart,respiration and brain function.If brain does not function and still heart and respiration are there we call it a coma stage but not death.In addition to it if voluntary respiration stops but maintained mechanically and heart continues to beat still the person is not dead.And after that if heart stops we declare him dead.Again we say the person is clinically dead because cells live longer .That is the reason how transplant of organs possible.There is one state called suspended animation mostly a YOGIC activity ,described in India, where the person does not have any of the above described three parameters to think somebody is living. So the dilemma is how to say definitely some body is dead and basing on which parameters? It is said that electroencephalography (EEG)should not show any electrical activity of brain to be declared as dead.That not feasible everywhere.Again even with that cells continue to live about 6 hours and if organs are preserved the longevity of cells increases even up to 24 hours and longer.So what is death and life ? It is something seen macroscopically and also can be seen microscopically.Here the question is what caused the life to cease in the macroscopic form and maintained in the microscopic form for some time, to cease totally later?
Is it that the respiration i.e. oxygen in air is important or the the functioning of heart and brain?What caused these parameters to cease functioning properly? What regulates these? We know regular electrical activity is going in side body for proper functioning of different parts and organs.But who regulates it.Obviously many will answer it to be brain.But we can not declare death if there no brain activity but heart and lungs functioning and what about above described suspended animation.
In India YOGIS said something PRANAMAYA SHARIRA,Meaning life filling everywhere in the body.and that is true I think.The regulator center for these the electrical or electromagnetic activity resides somewhere say in brain the master of body.Now can we call it SOUL ? If so is it a group of cells or something else which has a free access to out side and again right to return to body ? Can it be an electromagnetic force as we give some imaginative name, The Soul to it.Is it possible that there is some mega center somewhere else, as we are a part of this unimaginable Universe and name it GOD? And is it that the soul the brain center for life is pre-timed for destruction in individual cases? But can be boosted to some extended time with healthy habits? And is it that an electromagnetic force The Soul is withdrawn from some wearing body after a certain time period and put somewhere else to make that live before birth only to be withdrawn again later?
This is to some extent mixture of science and spirituality.And some thing described in" The GITA "in India.
So many questions, is not it? Again life and death remained mysterious. unthinkable and unsolvable for me by the time I returned home.My download of updates for PcLinux was still not complete and I started writing this thought.

HERPES A MISDIAGNOSIS/AND ?ONLINE VIRUS DEACTIVATION

Herpes are two types, namely herpes simplex and herpes zoster.Zoster is more often associated with Chicken-pox.Simplex usually affects the peripheral sensory nerve endings .It appears first with severe burning type of pain on some part of body usually around the face, there after reddish patches with small blisters.All along it is very painful. It became hard for me to diagnose one patient when it was involving almost one thigh with many reddish patchy eruptions and pain without any blisters.I was confused.Whether it is herpes or urticaria because the patient had taken some sea foods last night.But sceptically I diagnosed it to be allergic urticaria because urticarial rashes usually occurs almost all over the body but local involvement also seen and preferred site being around the face.After probably two days the blisters appeared making the diagnosis clear.Not many effective are available.Patient has to suffer from discomfort for some time in spite of the available treatment.Most distressing aspect is what is called post-herpetic neuralgia.This is a sort of discomfort and some times severe pain patient experience following healed herpes eruptions. Again not much medical help can be rendered except some symptomatic relief.Herpes of uterine cervix is sometimes blamed for future cancer cervix.
Having said this I propose that the computer viruses spread through internet.I think it is something like viruses in blood circulation in animals affecting target organs.As agents as drugs administered to fix them in the circulation ,will it not be possible to send some sort of anti virus agent to neutralize them in the circulation i.e. in the network itself .I hope this is possible.And this will help the end users to save on cost of anti virus products and ultimately national saving.

CAT THE MYTH/ DISEASE/BENEFITS

First I thank English muse whose black cat prompted me to write this article to-day.It is a tradition in this part of our country India that not to go ahead in journey if a cat crosses the road in front of some body because it signals bad luck.I have read in a Daily news Paper abut the myth surrounding it.Earlier times cats were worshiped somewhere in Egypt or Italy and the punishment for harming the cat was more severe in form of cutting the limbs or even death. That was why commuters prefer not to go ahead while a cat was crossing the road to be doubly sure it was not harmed .And that tradition is maintained here in India a in form that if somebody goes ahead ignoring the cat crossing the road he is going to meet an accident.
Personally I do not obey it and not faced any accident either.But domestic cats do a lot of good in India by feeding on rats and acting as a scavenger. Researchers have found that association with cats from childhood may prevent allergic diseases.Having said this I have to mention that cats are source of some diseases like cat scratch fever and can sometimes be a cause of the dreaded rabies if not vaccinated against it. It is said That "If you have not seen tiger see cat" as it comes under the family of tigers.

WONDERFUL WORLD AND MYSTERIOUS LIFE

It is dawn of Good Friday and a holiday in most part of the world.As usual I started my walk at 4.45AM under the setting moon and gentle breeze.I had already set my PC to download a movie.Again the wondering thought prevailed on me as my walk progressed towards the seashore.That was"In the search of God".Truly speaking personally I feel that we do not know what is life and who created us.While  resuscitating  dying patient I understand nothing about what is causing the patient to die .There is a very small fraction of difference between life and death.He was living now and dead within no time.We assign it to a little known cause i.e.Heart attack,Kidney failure, Respiratory arrest and excessive bleeding etc.But why this happens?Why there is a failure?We again assign it to some ill-defined cause.I think for a deterioration in the status of life is pre-defined and pre-timed. At a point  time some switch is made on to start the deterioration process.Is gene is the God?There has been tremendous work in this field.What little is known to me is that in the normal wear and tear process the genes get injured and the repair process is no more effective.If that is true what happens to the repair process ? Again mind can not calculate it.
If we and our earth are a part of this vast universe there bound to be similar conditions existing in some  other part of it.Can anybody even imagine where this vast universe ends and what is beyond that.I think there is no answer to it.
What makes the planets and stars so called magnetic.Why the system obeys certain pattern?
Is that something not known to our imagination guiding everything?
By this time about 45 minutes passed and I was back in my residence with so much sweating and un solved ,un imaginable and never ending quests.I saw my PC has downloaded only 35% of the movie while I started writting this.

EPILEPSY AND PRECAUTIONS

Epilepsy is a disease characterised by various types of convulsions starting from mild to more severe attacks.It may be idiopathic i.e. without any demonstrable abnormality in the central nervous system or secondary to certain diseases , more dangerous being brain tumours or other space occupying lesions. It is said that convulsive disorders seen before age of 6 years and after the age of 30 years should always be seen suspiously i.e. might be having some causative factor .So these cases should be investigated thoroughly befor leveling them to be idiopathic.If secondary to a disease attempt should be taken to cure it ,of course the experts in this line are to decide.In idiopathic ones medical treatment is the main stray of treatment.However surgical treatment for these groups are also being undertaken .More  importantly moral family support and strict following of precautionary measures are required.

CONTRACEPTIVE PILLS

Contraceptive pills are mainly hormone based and mainly used by women.these hormones change the uterine environment and some other parameters,so that conception and embedding of product of conception does not take place.These pills are safe when taken after consulting a doctor .Women suffering from diseases of breast ,thyroid,venous disorders, cardiac problems,liver diseases and kidney problem to name a few should positively consult their doctors.It also should be discontinued for some time according to the advice of doctors.

PHIMOSIS AND AGE

Many times children are sent to me to examine the possibility of phimosis in them.Phimosis is a condition of male where prepuce, the fore skin of penis could not be retracted back.And the urethra as a result could not be visualized.Actually it causes a concern in their parents.Normally it could not be retracted till the age of 2 years and I extend is up to 4 years.By which time everything becomes normal.So point is that non retraction of penile skin is normal till the age of 2 years and added to it we can wait for another 2 years to level it as phimosis. Some phimoses can be tackled by simple dilatation of the fore skin opening and all others by a minor surgery as a day care procedure called circumcision.

DIABETES MELLITUS AND PREPUCIAL INFECTION

Prepuce the fore skin of penis sometimes gets infected called prosthitis.There occurs dischage of pus from below the prepuce ,soreness ,pain and local swelling.This is very common in un-diagnosed and under or poorly controlled diabetics.So in these cases  plasma blood glucose estimation,both fasting and postprandial are advised.If diabetes is found has to be controlled alongwith antibiotics for infection.

BREAST MOUSE/INNOCENT TUMOUR

Mouse wanders always and more so if checked.One innocent tumour of female breast behaves in that manner inside the breast tissue and so is the name.There occurs  usually a small mass sometimes larger one inside the breast tissue,firm to feel,usually painless and moves here and there when palpated.Occurs in young adolescent and towards menopausal ladies but age is no bar.It is seldom dangerous but fine needle aspiration cytology should be done.Formerly it was called fibro-adenoma but now-a-days classified under the group of benign diseases called the Aberrations in Normal Development and Involution(ANDI).This means there occurs some aberrations in the normal development of breast during the adolescent period and normal involution towards the end of reproductive life.This can be left as such if too small i.e.<2cms and can be nicely removed under a local anaesthesia as a day care procedure.Larger ones called Giant fibro-adenoma may sometimes require total removal of breast tissue.And always the fully removed tumour should be subjected to histo-pathological examination to confirm the benign nature because sometimes something more dangerous might be hiding.Even if so it can be an early detection.So it is advised to go for self breast examination time and again.

We in orissa are time and again hit by cylcones,tornadoes and floods.Many lives are lost and loss of wealth is a lot.The mantal as well as physical health are deranged sometimes beyond recovery. But our Govt perhaps does not understand this.It is leanrt from "The Times of India"that there is no modern gadgets in meteorological dept. DIABETES MELLITUS TYPE 2 /ORAL HYPOGLYCEMIC AGENTS

Type 2 diabetes mellitus is often treated with drugs called oral hypoglycemic agents.There are vast number of drugs in this category.After taking the medications the blood glucose level is to be checked again to see that glucose level normalises in blood.When it normalises and newly diagnosed diabetics often think that they have been cured of diabetes and discontinue the drugs.In a number of occassions I have seen this.Also sometimes as to-day some patients come to ask whether to continue the drugs. So the fact is that the normal blood glucose we see is as a result of effect of drugs those continued,not without the drugs. So how can we discontinue the drugs? Of course after taking other measures like weight reduction in obese and life style changes sometimes one may not need it. TOBACCO/GASTRITIS

Tobacco is CONSUMED in many forms starting from raw to processed.In India it is added to limed beatle leaf along with beatle nut called PANA and chewed frequently.This chemical ladden fluid causes local changes in stomach and might lead to gastritis and ulceration in the inner layer(mucosa) of stomach.Recently one patient underwent endoscopy for pain in upper abdomen and dyspepsia having a history of tobacco chewing habit of 10 to 12 times a day. There was erosion of stomach mucosa.She was advised medications along with an advice to stop beatle.But she displayed an indifferent attitude towards the advice of doctor.I think as she was a PSU employee and had free access to medical doctors and supplied free medicines she wants to continue the bad habbit and at the same time would take medication to cure the ailment.This is what I call PSU mentality of patients.
