WORLD TOBACCO AWARENESS DAY /31st MARCH

The Supreme Court  of India has to intervene for the  publication of danger and awareness pictures/logos on packets of tobacco.The political and Govt. will is evident from this .This is a sort of habituation and can be given up with a little application of strong will power.Now-a-days it is not that people are not aware of the consequences of tobacco chewing.But everybody thinks that somebody else may become the victim,  not he.And he adopts a convenient position to defend that habit.They will argue why X is not affected in spite of his long standing habit.Again why Y become a victim without this habit.
But when affected the time has already gone out.Of course early dectection and treatment can cure the disease but what about the disfigurement.
I propose why at all products should be prepared from it and why can not production be stopped in stead of costly advertisements costing the exchaquer.I think it is only lack of will to stop production and aim of revenue generation  that  matters.

Recently I have read an article that insulin can be produced on the tobacco  leaves with genetic modification.If that materializes the ugly face of tobacco will be a thing of past.

PRE/POST EXERCISE DRINK

Exercise generates heat in the body and it is cooled off by sweating.My place being a coastal area there is a lot of humidity resulting in excessive sweating .That causes a lot of  water and salt loss from body.There has been a number of suggestions for pre and post exercise rehydration drinks.I follow a very simple, priceless as well as healthy drink.As I do, early morning exercise is very good as pollution level is low and morning ultra-violet rays help in synthesis of Vit.D.I start walking after taking about 500ml.of plain water.After a brisk walk of 50 minutes I again take 300 ml of plain water  and go for a weight bearing exercise at home for 10 minutes .There after I drink 750 ml. of water to which added a pinch of common salt and juice extracted from a slice of lemon. That effectively fills up my water need.Additionally some can add about 10 grms. of glucose, if not diabetic. There after I take a cup of tea and sprouting green gram .Breakfast is taken a little later and I am ready for hospital.

MEDICAL STUDY ,A STATUS REVIEW.

At present entrance tests are going on in India for entry into professional courses like MBBS,Engineering and MBA etc. after 12th class.I read a good article in the local daily"The Samaj" and wanted to give my own comment.It has been described that students are preferring engineering stream over medical and I agree to it.In India medical service in Govt. Sector is not satisfactory.Now it is not considered to be a noble profession also.This is a set back and no body seems to be concerned about it.Time will come when this profession will only get entrants from back benches.
Getting to medical course is a difficult affair in a sense that one has to be thorough in pre-medical subjects.The time of study is longer than engineering and the study never ends i.e. one has to continuously go on updating himself through out his life.The dissatisfaction and indifferent attitude of general public towards the doctors has to be borne .The odd time of work i.e. at any time one can be called to attend patients.The false acquisitions by the patients  hurts  many times .Lack of infrastructure for professional growth i.e. even there is no roof for a doctor at some places.Good schools for study of their children is a day dream at many places.  And as a final nail on the coffin of aspiration to become a doctor is abysmal salary in comparison to engineering and MBA graduates. All these disadvantages are gone if one opts for engineering or MBA.
That is why there is brain drain from India.
I think time has already come in India for think thanks to explore possibilities for medical profession to grow in knowledge and change the outlook towards the profession. It is also true that along with general degradation in social value there has been degradation in this profession also.
BUT EXCEPTION IS NOT THE RULE.

YOGURT AND LACTIC ACID BACILLI

Body has it's own defense mechanism in this world of bacterias.Harmful bacterias are present every where so also on the whole body.They are also ingested by way of food and water intake.A drop of water may contain millions of bacteria and they have the potential to multiply very quickly.It is not that we fall prey to them in contacting infections frequently.There is a host of defense mechanisms to prevent these.
One of them is good and body friendly bacteria called lactobacillus .They are normaly present in the intestine .mouth and some other parts of body.They protect body by preventing colonisation of harmful bacterias inadverdently shallowed during taking food.They also produce some B group of vitamins required for body.They get destroyed by taking antibiotics for cure of some other infection.Normaly they recover.But many a times physicians suppliment it by providing lactabacilli from external source.
  Yogurt is a good source of bacilli.

REST/FOOD/FREE RADICALS

In this materialistic world everybody runs and runs .We all are exhausted in the long run.Rest the word is long forgotten as well as entertainment. Body being a machine consisting of blood ,flesh and bone etc. requires rest as well.In the busy life style body accumulates a lot of toxic substances one being free oxygen radical. This one is potentially harmful if not scavenged.Body has it's own mechanism to tackle this but rest is the main factor for it.Proper food contains factors called antioxidants to scavenge this.Proper foods,I mean a balanced diet enriched with green vegetables,red/dark skinned/rich colored fruits and sprouts etc.
Rest means adequate sleep and that rejuvenates body by eliminating harmful toxic by-products of metabolism and the free radical scavengers viz.Vit-C,E,A and trace elements like selenium help in this process.

Grapes,carrots,sprouts,mango and even dark vegetables contain a lot of antioxidants.Marketed preparations are available but why not get it from food.

ACCESSORY BREASTS/NIPPLES IN FEMALE.

Accessory breasts and nipples in female are more of disfigurements than disease.These are developmental errors.In animals(including human) there develops one milk line stretching from the axilla to the abdomen on each side of body in a female fetus(while in the mothers womb).From these milk lines five to six rudimentary breasts with nipples formed to develop further to future full fledged breasts on either side of body.In lower animals these persists in form of more than one nipple to develop further in the later life for lactation  as seen in dogs and cats.But in human all the nipple disappear to the time of birth leaving only one on each side of body on the chest to develop further in the reproductive life.Errors do occur.And sometimes more than one nipple remains on one  or both sides and may develop further to lactate .Also some times  an accessory breast also develops from the remnants in the milk line usually in the axilla without a nipple as a breast.It may enlarge further to cause pain called mastalgia or engorge with milk during lactation period.
These more of disfogurement and discomfort.And necessiates cosmetic removal and repair.

BREAKFAST/GALL BLADDER STONE AND OBESITY

In India many skip breakfast for various reasons.Ladies specially skip it with a plea that they are busy in household work.These persons many a times suffer from gall bladder stone,obesity and diabetes.
   After the over night fasting ,in the morning body needs energy to start the day's work.The gall bladder stores and concentrates bile.Bile secretion is stimulated by intake of food and particularly mixed with fatty food.During the night as there  is a fasting state and it remains stored.When one takes breakfast it is empties to mix with the food to help in digestion of food particularity fats.Skipping breakfast keeps the stored bile for longer period of time and in the long run it may get infected to form particulates on the bacterias initiating stone formation.Also it is likely that the concentrated bile is prone for particle formation as such.That bile to fat ratio must remain within normal range.
   Skipping breakfast prompt the individual for interval eating which may fall to a habit ending up in excessive food intake as well as calorie. This in is tern reflected in making the individual obese and the chain reaction of diabetes,hypertension and the kidney diseases etc.

   So it is ideal to start the day with a good balanced breakfast.In our part it is told that 'take the breakfast like a prince,lunch like a king and night meal like a beggar'.But here there is rise in incidence of gall bladder stone disease because the saying is seldom followed.

OSTEOARTHROSIS OF JOINTS

Usually the weight bearing joints are affected ,the most common being the knee joint followed by ankle and hip joints.There is lot of pain which commences when the joint is first used then to subsides a little but as disease progresses all the movements become painful.So many causes attributed to it.Weight gain and excessive wear and tear of joint cushion due to aging are much important.X-Rays evaluation reveals changes after a period of time and blood sample tests for other collagen diseases usually negative.
Patient's satisfaction on treatment is not easily achieved as they heavily depend on medicinal treatment.But mainstay of treatment is weight reduction in case of obese and physiotherapy which patient does not comply to.
Some cartilage regenerating molecules ,methylcobalamin ,simple analgesics and physiotherapy with life style modification should give satisfactory remission. Interventional treatments such as local injection of steroids and other molecules can be tried.As a last resort knee replacement by synthetic materials can be done.

As habituated in surfing internet and firefox addons ,I found a new utility called zemanta which automatically finds out tags for blogs, to call the blog as zemified.One may include a wiget for donation towards a noble cause.You yourself can see my site and understand.It also provides different pictures to be inserted into the blog.
I am very happy with it's performance.It provides news for improvement in blogs.I could see now a news for 15 useful firefox extensions for bloggers.I shall go and check.

SLE/ MISDIAGNOSIS AND COMPLICATIONS

It was only an alert to me some 18 years back that my brother-in-law was suffering from dry fever for about 5 to 7 days.At that time I was in a primary health center in a tribal belt of Orissa,India and recently completed my graduation trying for post graduation.Again after 4 to 5 days message came that the fever was continuing in spite of treatment for common causes of fever .One of them prevalent in the area was malaria.So I reached their place traveling about 200 Kms.I saw him and except dry fever no other symptom or sign was discovered and was on anti-malarial therapy. Hoping a recovery I returned to my PHC to receive again a message that there had been no remission.So I suggested hospitalization in the SCB medical college,Cuttack some 40 Kms from his place and reached straight there.My co-brother from Tatanagar also joined so also other relatives.My teachers were there and all relevant investigations for fever that has crossed the dead line of 6 weeks to be labeled as fever of unknown origin were being carried out.Only some non specific minor abnormalities were discovered without significant contribution towards a correct diagnosis.
Again some presumptive treatments given and he was discharged from hospital to come back again some 4 to 5 days later.In the meanwhile I got transferred to a nearer place to the medical college . By this time he had developed collection of fluid out side his lungs in the chest.The common diagnosis for this condition in this part was uberculosis and treatment starts without waiting for other confirmatory investigations.The fluid aspirated was bloody in nature giving a suspicion of cancer and was sent for cellular test to two different pathological labs.
Both the places reported that to be oat cell carcinoma of lungs a dreaded one having poorest prognosis.We were shocked.He was married only 3 to 4 years back blessed with a girl child.
The suggestion came up was to take him to one of the best center for cancer research and treatment in Mumbai(Bombay then) The Tata Memorial Center.We travelled more than 1000 Kms in a much pitiable condition to reach there.There the clue, the collected fluid was tested again and to much relief no cancer cells were found .So the cancer diagnosis was abandoned and once again search continued for a diagnosis. Presumed to be tuberculosis anti-tuberculous drugs continued which had already been started.One thing that was done there was addition of steroids.
Much to our relief the fever subsided slowly and we were happy.But our happiness did not last long as fever again re-appeared when the dose of steroid was tapered to be stopped finally.But one thing came up is that the disease was steroid responsive and the commonest was one of the collagen diseases.But all available common investigations at Cuttack had already been exhausted.So he was shifted to Tata Main hospital ,Jamsedpur ,my co-brother's place where the antibody against double stranded DNA was tested positive branding it again a bad disease, Systemic Lupus Erythematus.And steroid was the drug of choice.Then we could co-relate how the fever subsided with addition of steroid at Mumbai to anti-tubercular drugs .
He did well for some years ,there after only to damage the ligaments at knee joints which were repaired.Again he developed steroid induced cataract in both the eyes to get operated for.A long time passed, to our misfortune SLE activity flared time and again to be suppressed by increasing doses of steroid.Finally immune suppressant chemotherapy was added and he is doing fine till date.He is under the treatment of Dr Roy of Tatamotors hospital at Jamsedpur.
This live story describes in a nutshell how a disease can behave in an atypical manner, can be mis-diagnosed as cancer , the complications of disease and treatment , the relapses and control .

SOME SOFTWARES /PC HEALTH

To-day I could not make it a point to get the daily dose of walking.But I shall make it up in the afternoon perhaps.Or else I am completing walking 5to 6 times a week at any rate.
So I sat before my PC,went to filehippo update checker and could not find any updates.Instead I landed up in downloading one system protection application scotty on patrol ,a-squared free 4.5 spyware and malware guard and remover application and one encryption (flyline) software Trucrypt.Then I updated spyware terminator by downloading a fresh copy as my existing one did not complete the updates.I again integrated it with Clamv antivirus,systemprotect and web shield but did not like the vault.

Previously I had registered with Zemanta and added to my blog ,I practically saw it's wonderful action which automatically selects levels and tags besides providing colorful pictures .It is worth trying. Called zemified.

ENLARGED PROSTATE AND IT'S COMPLICATIONS

passage passing through the prostate.
 



The innocent enlargement of prostate is called benign enlargement of  prostate(BEP).This is a gland somewhat the shape of an apple(Not size) usually 15 grams to 20 grams in weight .Through it passes the urethra starting from the urinary bladder called the bladder neck.It's function is not very clear but believed to facilitate and nourish the sperm during their travel inside female reproductive system.
  Though there are various postulates forwarded, the most commonly accepted one is the imbalance in male and female hormones after the age of forty.As it grows in size pushes itself up, elevating the bladder neck and narrowing and squeezing the urethra passing through it, to cause symptoms of urinary obstruction.This enlargement being natural phenomena one need not worry if is not getting any symptoms.That is an asymptomatic persons need not worry except periodical ultrasonological  evaluation.
 A few symptoms had already been described last time.An apparently asymptomatic person may get a retention of urine after taking alcohol and long bed ridden patients may also get it .This is an urgency and has to be relieved by catheterization .
Other complications are more serious in nature.There is back pressure of stocked urine inside  the bladder on the total urinary system above i.e. bladder itself,ureters,kidneys to create problems .And once kidneys are affected  systemic disorders like impairment of brain function occurs.Infection in the stocked urine is a constant problem and gives rise to stone formation .There may be out pouching of bladder at places creating additional pockets in the bladder called diverticula, constantly keeping infected urine inside it and promoting stone formation.The ureters and kidneys may get dilated from minimal extent to a great extent jeopardizing the kidney function. And that may lead to most dreaded kidney failure.There may occur bleeding inside the bladder.
  Very good treatment options are available .I think if the patient is not frail and not unfit for anaesthesia transurethral resection of prostate (TURP) is a very good option.And if frail, medical treatment and catheterization are to be followed with treating physician's advice only.

ENLARGED PROSTATE AND BLADDER OUTLET OBSTRUCTION

Usually a man after forty comes to the hospital writhing in pain, leaning forward to get some relief and complaining of inability to pass urine for sometime (Retention of urine).Most commonly is suffering from bladder outlet obstruction in form of enlarged prostate.Prostate is an organ situated just below the outlet of urinary bladder.After age of forty when all other organ function recede this organ used to grow in size.This causes obstruction to passage of urine.Many a times the beginning of it's growth has been warned earlier in form of frequent night urinations,desire to pass urine urgently, a few drops of urine escaping before finding a suitable place to micturate,inability to hold urine if required and long patient waiting time to pass urine  etc.
  If a male person is facing these difficulties should get examined for disease of prostate which is more frequently an innocent enlargement to at times a cancerous one.This is very acurately diagnosed by an ultrasonography.Among  blood sample examinations, prostate specific antigen is most important.X-Rays examination of bones are also done to exclude malignat deposits.For innocent ones treatment options varies from medical to surgical depending upon the patient's fitness and a host of other factors.But nice treatment is possible. Maligant prostate also can be managed satisfactorily as the condition is very slow progressing one.

TENNIS ELBOW

Tennis elbow is more of a discomfort than a disease.Here there is pain in the outer side of elbow while attempting to hold something or trying to work in the affected hand.It may go to the extent of dropping things from the hand.Usually affects the hand of tennis players   which hits the ball and so is the name.But can affect others using the hand in the same way.Here a long duration of traction force generated from the extensor group of muscles at elbow acts on the  lower end of arm bone and adjoining forearm bone to set the pain in motion.Patient  must be checked for associated diabetes.

  It can be relieved by rest to the part ,moist heat application and use of simple analgesics.If not, steroid injections are given at the site.And if does not respond one may think of taking up surgical remedy.
But usualy it subsides with the simple measures.

G.I. ENDOSCOPY.

Endoscopy is a procedure wherein an image capturing flexible device is put into the system to be viewed.In gastro-intestinal (G.I.) endoscopy ,the device called endoscope is put into stomach through mouth for upper G.I. and to colon through anal opening for lower G.I. endoscopy.
There is minimal discomfort only.The inside of the intestine is viewed for common diseases like ulcers,tumors and varices etc.Those can be viewed nicely and video or stills images can be captured .
This has made diagnosis of early cancers of stomach possible as compared to the earlier times.Tissue can be taken for examination for confirmed diagnosis.
Therapeutic procedures for certain diseases can be carried out through it.Most common procedures are treating bleeding ulcers,ligating esophagial varices and polyp removal.endoscopy

MAY DAY AND SAFETY IN WORK PLACE.

As we know 1st May is observed as "Worker's Day".Many things are told and many lectures are delivered as usual to be forgotten towards the evening.As a surgeon in a Port Trust, a labor oriented organization I have to manage the work site injury cases.I find a good number of injuries are due no use of safety protections/precautionary measures.The most common being no use of shoes, hand gloves and the top most is lack of attentiveness.Though strict measures are taken by the organization for the use of those items somehow they manage not to use those.
     So important is self consciousness in the part of the workers.
            Having said this ,I remind that this day is primarily observed for victory of workers in limiting their working hours to 8 hours a day.I think there are sectors where it is better said than followed e.g. software professionals in MNCs and some private sector organizations.
   The watchdogs are there but are knowing ignorant.The politicians infamous for their speeches only hardly have a truthful desire to do anything concrete.
